A rewarding opportunity is available for an experienced and compassionate occupational therapist with a background in home health services. This role is focused on supporting patients within their homes and helping them regain or maintain independence in daily activities while ensuring safety and maximizing their quality of life.

Schedule/Hours:

  • Full-time, 13-week contract
  • Monday through Friday, 8:00 AM – 5:00 PM
  • Expect approximately 25–32 points per week (5–7 patient visits daily, depending on visit types)
  • Key Responsibilities:

  • Conduct comprehensive occupational therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge when applicable
  • Develop and implement individualized treatment plans centered on ADLs, IADLs, upper extremity rehabilitation, cognition, and home safety
  • Deliver skilled interventions including therapeutic activities, self-care training, adaptive equipment instruction, and environmental modifications
  • Recommend appropriate durable medical equipment (DME) based on patient needs
  • Continuously assess progress, revise plans as needed, and communicate effectively with physicians and interdisciplinary care teams
  • Educate patients and caregivers on functional independence, fall prevention, and compensatory strategies for daily living
  • Accurately document all services and progress in the Home Care Home Base (HCHB) system
  • Attend case conferences and professional development meetings as required
  • Maintain compliance with agency protocols, state and federal regulations
  • Qualifications:

  • Active, unencumbered Occupational Therapist license in the state of practice
  • Minimum of one year of recent experience in home health OT (field-based experience strongly preferred)
  • Proficiency with OASIS and Home Care Home Base (HCHB) preferred
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to function independently and manage a variable caseload
  • Strong communication, organizational, and time management abilities

    $2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
